Consent Form

This form MUST be read by parents and legal guardians of under 18 participants Tree House Trek - Activities on the course can be physical, outdoor, all weather, challenging activities at height, which have inherent hazards associated with them. Whilst Tree House Trek (THT) takes all necessary precautions to try and ensure the safety of all participants, unfortunately accidents, trips or collisions may occur as a consequence of taking part. Tree House Trek accepts no responsibility for any loss or injury resulting from any person’s involvement in THT ‘I’ understand that Tree House Trek accepts no responsibility for loss, damage or injury caused by or during participation in the Activity, except where the loss, damage or injury was caused directly by Tree House Trek’s negligence. It is understood and agreed that individuals participate at their own risk. As the responsible adult; I acknowledge my responsibility and will ensure that myself and/or the under 18 Children and Young People (CYP) under my care will be made aware of the responsibility that they have to follow instructions and behave appropriately. I will make staff aware of any medical conditions that myself or any of the CYP under my care have, which might have an adverse affect on their ability and safety. I also confirm that I will inform staff of any specific needs. If I am not the parent/legal guardian of any CYP under my care, I have described the activity and risks and have authority from the parent or legal guardian for them to attend and participate in the activity. I have explained to any under 18s with me the importance of listening to the instructors and following all rules and procedures. I have also made it clear to them that they should not start the Activity unless they understand all of the instructions given. You will be asked to confirm that you have understood and accept all of the above in formation prior to commencing your experience with us.

The ‘Challenge by Choice’ approach means that your child will be encouraged to decide if, when, and how far they step outside of their ‘Comfort Zone’, to take on a challenge - without coercion from staff and peers.‘I’ understand that Tree House Trek accepts no responsibility for loss, damage or injury caused by or during participation in the Activity, except where the loss, damage or injury was caused directly by Tree House Trek’s negligence.I give permission for my child(ren) to participate, I acknowledge my responsibility and will ensure that my child(ren) will be made aware that they must follow instructions and behave appropriately. I understand that some of the ground-based activities, bouldering and the obstacle course may be undertaken without direct supervision, following the safety instruction. I accept that, if my child(ren) are not compliant with the rules of the Centre, then they may not be able to take part.I will make the trip/party organiser aware of any medical conditions that are relevant to their ability to safely undertake and complete any of the Tree House Trek activities, I also confirm that I will inform the trip organiser of any specific needs. I agree to the trip organiser sharing relevant medical information about my child with the instructors.
